Good planning makes the difference.<\/p>\n<h2>Before discharge<\/h2>\n<ul>\n<li>Ask the ward for a discharge plan and a list of medication<\/li>\n<li>Ask whether reablement or intermediate care is available<\/li>\n<li>Check equipment has been delivered<\/li>\n<li>Arrange who will be there on the first day<\/li>\n<\/ul>\n<h2>What home care can do<\/h2>\n<p>Short-term home care can help with washing, dressing, meals and medication while someone regains strength. The aim is to build independence back up, then reduce support as the person recovers.<\/p>\n<h2>Watch for warning signs<\/h2>\n<p>Confusion, not eating, new pain, breathlessness or signs of infection should be reported to the GP or NHS 111 straight away.<\/p>\n<h2>Preparing the home<\/h2>\n<p>Before discharge, make sure the heating works, there is food in the house and a bed is set up downstairs if stairs are a problem. Move rugs and clutter that could cause falls.<\/p>\n<h2>Medication after hospital<\/h2>\n<p>Medication often changes during a hospital stay. Make sure old tablets that have been stopped are removed and that everyone understands the new doses. Our care workers follow the medication plan and record every dose.<\/p>\n<h2>Reviewing support<\/h2>\n<p>As strength returns, visits can be reduced. We review support regularly so people receive the help they need and no more.<\/p>\n<h2>Frequently asked questions<\/h2>\n<h3>How quickly can care start after discharge?<\/h3>\n<p>Tell us the planned discharge date as early as possible. Where we have capacity, we can often start on the day of discharge.<\/p>\n<h3>Is post-hospital care free?<\/h3>\n<p>Reablement arranged by the council is often free for a limited period.
