What to include
- The person’s name, address and date of birth
- A summary of their needs and any risks
- Preferred start date and visit times
- Funding arrangements, if known
- Contact details for the referrer and next of kin
How to send it
Download and complete the referral form from our referral page and email it to referrals.support@cevero.co.uk. For urgent referrals, call 01332 949721.
What happens next
A care coordinator reviews the referral, contacts you to confirm details and arranges an assessment with the person. We then agree a care plan and a start date.
Information that speeds things up
Recent assessments, care and support plans from other providers, a list of current medication and details of any equipment in the home all help us plan safe care quickly. Share what you can, securely.
Urgent and hospital discharge referrals
Tell us the planned discharge date as early as possible and call us for urgent referrals. We will confirm quickly whether we have capacity in the area.
Keeping you updated
Once care begins, we keep the referrer and family informed about how the person is settling in and flag any concerns promptly.
