Before care starts we visit to understand the person, their home and what support they need. It is a conversation, not a test.
Who should be there
The person receiving care, and anyone they would like with them, such as a family member or friend.
What we talk about
- Daily routine and preferences
- Personal care, meals and medication
- Health conditions and any recent changes
- Mobility, falls and equipment
- What matters most to the person
Risk assessment
We look at the home to keep everyone safe, for example access, lighting, trip hazards and equipment.
The care plan
We write a care plan with the person, setting out visit times, tasks and how they like things done. It is reviewed regularly and whenever needs change.
How to prepare
Have a list of medication, contact details for the GP and family, and any letters from hospital or social services to hand. Think about what a good day looks like and what help would make the biggest difference.
After the assessment
We send you the proposed care plan and costs in writing. You can ask questions, suggest changes and take your time to decide. Once you agree, we confirm a start date and introduce the care workers.
Reviews
We review the care plan after the first few weeks and at regular intervals, and straight away if needs change.
